
Hope 4U delivers holistic advice and support to individuals and families who find themselves in a vulnerable situation.
In partnership with the Cadent Foundation, Hope 4U work directly with the University Hospitals, Queens Hospital Burton and the Royal Derby Hospital delivering effective advice and support to improve the lives and housing conditions of patients treated for a respiratory illness.
The NHS Impact+ teams work directly with patients suffering with a respiratory disease with their primary base at the Florence Nightingale and multi pulmonary clinics throughout the areas.
Hope 4U support identified patients ensuring that their homes are warm and energy efficient, identifying any unclaimed benefits and applying for additional grants and support.
Working closely with the NHS, the added wellbeing benefits and wider implications will be identified, such as, reduced number of visits to the GP, hospital admissions and long-term medication use due to additional available money into the household to support with heating and nutritional food.
As a new partner, the first 12 months of the project will see 1,000 referrals received with outcomes including Priority Services Register sign ups, carbon monoxide awareness, benefit checks, energy behaviour change and other grant savings. Small energy saving measures are also to be distributed.
